Not very realistic.Any nation with the capability of long range attack would use a fusion weapon.On top of that it would be an air burst.Manhattan would simply cease to exist.

Using missile delivery would reveal who launched the attack and invite retaliation. Use a sneak attack, like smuggling warheads on cargo ships, and there’s a good chance the US wouldn’t be able to retaliate or even retaliate against the wrong country.
