Isn’t that the blank firing adaptor/miles gear on the muzzle of the US advisor’s M4?

He and the two guys next to him seem to have miles sensors on their helmets.

They are ISF Romanian troops. Their main battle rifle is the Kalishnikov. The only one not carrying a Kalishnikov is the American soldier in the ACU camo. You can clearly see his M4.
