Looks like there was a memory paging fault and the system can’t refresh the desktop to reflect translational movement of windows.
Someone took advantage of that fact and made the pseudo-3d snake of IExploder failure windows.

Oh we’ve all spent time playing with the cloning error message. This one really looks cool. Guess you can’t turn off artistic tendencies.
