Dreads often mount a flamer under the powerfist. Hard to tell from that angle, though.
Also, if you play Dawn of War, you can usually see a Dread squeeze some poor infantryman in the fist, hoist them high, and ignite the flamer before tossing them haphazardly over their shoulder. It’s probably my favorite melee animation in the game.

Not a multi-melta. Looks like a Plasma Cannon, as melta guns don’t glow blue.
