His powers grew out of control, and he ended up transforming into a pure energy being and relocating to another universe, blowing up Tokyo in the process. Tetsuo was a later, exponentially more powerful subject the military accidentally picked up and decided to experiment on because he had a similar genetic structure to Akira. He too grew out of control, but Akira came back and transported him to a new universe before he transformed so he didn't destroy this one when he transformed.
